N-Gen Rehabilitation LLC. is a mobile, fee-for-service physical therpay practice located in Northern Delaware founded by Dr. Louis Awad. N-Gen Rehabilitation LLC. specializes in disorders of the brain and nervous system - such as stroke, traumatic brain injury, parkinson's disease, vertigo, etc. - that impair a person's ability to walk and balance. Our therapists understand the significant impact such disorders have on our patients' quality of life. As a mobile provider of physical therapy services, we are committed to providing excellent, evidence-based rehabilitation in the privacy of our patient's homes. As a "fee at the time of service" provider, we are able to provide high quality treatments for our patients that are not impacted by changes in insurance company policies. We are also not burdened by high overhead costs, and are thus able to assist those who require financial assistance.
Prior to your initial evaluation: we recommend that you call your insurance company to fully understand your rehabilitation services benefits. At the bottom of this page is a helpful worksheet that you can use as a guide to the types of questions you should ask. After each therapy session, you will receive a written statement which you can provide to your insurance company for reimbursement if you qualify. Other documentation can also be provided at your request. Typical reimbursement rates vary between 40-80% depending on your insurance plan.
Only payments in the form of cash or check will be accepted. Arrangements can be made to pay by Visa/Mastercard, if necessary.
FEE SCHEDULE:
Initial Evaluation (Gait and Balance) - $150.00
Initial Evaluations (All Other) - $125.00
Follow up Appointments - $100.00
-A $50.00 fee will be applied to your account if you cancel less than 24hrs before your appointment time.
Chart Review and Over-the-Phone Consult - $50.00
